Collections Instructions: Patient must be wearing a valid armband containing name, medical record number, and date of birth. Forward promptly.
Note: Specimens labeled with CollectionManager must have with the patient’s full name, medical record number, phlebotomist’s initials, and date and time of draw. Specimens without CollectionManager labels must have with the patient’s full name, medical record number, and date and time of draw and the initials of 2 staff members that have identified the patient at the time of draw.
Agglutination
Includes ABO, antibody screen (indirect coombs), and Rh type. Can be ordered in preparation for infusion of blood products, but requires an armband.
Note: No units of blood are selected for patient with this order. Crossmatch of blood following a type and screen requires 5 to 10 minutes. Type and screen specimens expire in 3 days and can no longer be used for a crossmatch. Patient must wear armband during this time. If removed, a new specimen must be drawn.
